Adding Locations

Active MPOG sites interested in adding additional locations are required to provide background information for each proposed site.

Enrollment of Additional Locations

If you are an active MPOG site and are interested in adding additional locations, MPOG requires background information for each site you wish to include. This information allows the MPOG Coordinating Center to appropriately assess readiness, data integration needs, and compliance requirements. Please note that for sites located outside of Michigan, additional fees may apply. For detailed information regarding applicable fees, please refer to the Funding Requirements for Non-Michigan Hospital page.

When you are ready to proceed, please complete the MPOG Membership Application, which collects the information necessary to guide you through the site expansion process. Under the question, What best describes your request?”, select “We are an existing MPOG site and wish to add additional locations.” The MPOG Coordinating Center will review your application and contact you with the next steps once the review is complete.

Please do not enable or onboard any additional locations until you have received formal approval from MPOG.
